You are required to prove your eligibility when subscribing to LifeLine services.
If you are enrolled in one of the programs listed below, you automatically qualify to enroll in one of our discounted phone plans.

No one in my household currently receives LifeLine support through another phone carrier. If someone in your household is receiving Lifeline telephone service you must cancel the service before applying for LifeLine Service though Xchange Telecom.

You have a valid United States Postal Address. P.O. Boxes cannot be accepted.
The address is your primary residense, not a secondary home or business.
